| Dr Fonarow is professor of medicine at the University of California, Los Angeles (UCLA) School of Medicine. He serves as director of the Ahmanson-UCLA Cardiomyopathy Center and is director of the UCLA Cardiology Fellowship Program. His research interests center on heart failure management and implementation of treatment algorithms to improve clinical outcomes. heart failure and new therapies and management strategies to treat this disease state. He has authored the UCLA Clinical Practice Guidelines for heart failure, acute myocardial infarction, atherosclerosis prevention and treatment, and unstable angina. Additionally, Dr Fonarow developed and successfully implemented a comprehensive atherosclerosis treatment program at the UCLA Medical Center, the Cardiac Hospitalization Atherosclerosis Management Program (CHAMP).

| Barry H. Greenberg, MD |

| Dr Greenberg, professor of medicine at the University of California, San Diego (UCSD) School of Medicine and director of UCSD Medical Center’s Advanced Heart Failure Treatment Program, has a longstanding interest in the basic cellular mechanisms of heart failure and new approaches to treatment. He is a founding member of the Heart Failure Society of America (HFSA), chair of the Education Committee, and vice president of the organization. |

Dr Greenberg is co-editor of Congestive Heart Failure, the first comprehensive text in the field, and has recently edited Cardiac Remodeling: Mechanisms and Treatment, which will be published in the fall of 2005. He is an associate editor of Journal of the American College of Cardiology and has authored numerous articles in the heart failure field covering a wide range of clinical and basic science topics. Dr Greenberg has been recognized as one of the Best Doctors in America continuously since 1992.

| Dr Pitt is professor of internal medicine, Emeritus, at the University of Michigan Medical School in Ann Arbor, Michigan. Dr Pitt has been principal investigator or co-investigator on a large number of major clinical trials, among them AVERT, ELITE, EPHESUS, PLAC, PREVENT, QUIET, RALES, SOLVD, and TREND. |

Widely published, he is the author of more than 400 articles in peer-reviewed journals such as the New England Journal of Medicine, the American Journal of Cardiology, Circulation, and the Journal of Cardiac Failure. Before joining the University of Michigan Medical School faculty, Dr Pitt was associate professor of medicine at Johns Hopkins University School of Medicine in Baltimore. |
